Social Services
The Social Services department of Esperanza Health Center is designed to assist patients with the vital resources necessary for their overall well-being. It is also designed to be a resource for area churches and ministries. Esperanza's Social Services department works in conjunction with the entire medical interdisciplinary team to establish a safe and appropriate care plan to meet the needs of each patient. Assistance is available for, but not limited to, the following areas:
- Behavioral health services
- Substance abuse & drug addiction
- Prescription and insurance coverage
- Transportation
- Housing
- Welfare and Social Security assistance
- Advance Directives and end-of-life issues
- Domestic violence and other family issues
- Food and nutrition
- Utility assistance
- Dental care
Esperanza's Social Services staff members work with patients and their family members, offering emotional, social and spiritual support, and helping patients access other resources and assistance necessary for continuity of care.
